Access to public information
Form and procedure for requesting information
In accordance with the Law of Ukraine "On Access to Public Information" of January 13, 2011, No. 2939-ІI, the Decree of the President of Ukraine "On Measures to Provide Access to Public Information in Subsidiary Bodies Created by the President of Ukraine" No. 548 of May 5, 2011 / 2011 and for proper organization of access to public information, the administrator of which is the Committee approved the "Provision on ensuring access to public information in the Committee on State Prizes of Ukraine in the field of science and technology"
Public information is provided by the Secretariat of the Committee in response to a request for information.
According to the law, publicly disclosed and documented by any means and on any medium the information obtained or created in the course of the fulfillment by the subjects of authority of their duties, provided by the current legislation, or which is in the possession of the sub- objects of authority.
An individual request for information is made in an arbitrary form. It is necessary to specify:
1) the name of the applicant, postal address or e-mail address, as well as the telephone number;
2) a description of the information that the requestor would like to receive. (The type, name, details or contents of the document for which the request is made)
3) signature and date.
